Complete and sign this form if your child/children will be participating in the Annual Recital. One form per child.

Costume payment due – 11th July, 2026

By submitting this permission form, I confirm that my child/children will be participating in the recital and have read and understood the conditions for doing so.

I further agree to pay all costume costs by 11th July, 2026.
